All these are excellent questions. I wholeheartedly believe that it is a two-way dialogue. The company has to be ready to answer these questions too. The inability to articulate the answers to these questions = they are not prepared for the interview. The value or culture of the company is not clear, or they are hesitant to share.

I would add requesting to meet the team or walking the work floor. If you see people have lunch at the table, then take they as a warning.
